Understanding NCCI Edits: A Guide to Preventing Claim Denials

The National Correct Coding Initiative (NCCI) is a CMS program that prevents improper coding combinations. It flags pairs of procedures that should not be billed together (or require modifiers).

🔹 What Are NCCI Edits?

  • Purpose: Prevent unbundling (billing separately for services that should be combined).
  • Types of Edits:
    • Column 1/Column 2 Edits: Two codes that should not be billed together.
    • Mutually Exclusive Edits: Codes that cannot logically occur together.
    • Modifier Indicators: Shows if a modifier can bypass the edit.

🔹 How NCCI Edits Work

Code PairEdit TypeModifier Allowed?Example
99213 + 11720Column 1/Column 2Yes (-25)Office visit + toenail removal
12004 + 12005Mutually ExclusiveNoTwo wound repairs of same complexity

🔹 Common NCCI Scenarios

✔ Allowed with Modifier:

  • -25: Separate E/M service (e.g., office visit + injection)
  • -59: Distinct procedural service (e.g., biopsy + excision at different sites)

✖ Never Allowed:

  • Bilateral procedures without -50/-LT/-RT
  • Component coding (e.g., billing 93010 + 93000 together)

🔹 How to Check NCCI Edits

  1. CMS NCCI ToolsOfficial NCCI Policy Manual
  2. Encoder Software: Optum, AAPC Coder
  3. Payer-Specific Edits: Some insurers (e.g., UnitedHealthcare) have additional rules

🔹 Best Practices

✔ Use Modifiers Correctly (only when justified)
✔ Audit Claims Pre-Submission
✔ Train Staff on NCCI Updates (changes occur quarterly)
