What is an EOB?

An Explanation of Benefits (EOB) is a statement from your insurance company that shows:

  • What services were billed by your doctor
  • What portion insurance covered
  • What you may owe

How to Read It

  1. Patient/Provider Info – Confirm names and dates are correct.
  2. Service Details – Lists treatments (e.g., “Office Visit – $150”).
  3. Insurance Payment – Shows what they paid (e.g., “$100 covered”).
  4. Your Responsibility – Highlights your balance (e.g., “$50 copay”).

Why It’s Not a Bill

  • An EOB explains costs—your doctor’s office will send a separate bill if you owe money.
  • Check for errors (e.g., wrong service dates or denied claims).
